The Illinois Primary Health Care Association (IPHCA) has compiled a set of resources to enhance the integration of community health workers into the clinical health workforce. Resources include sample forms and scripts for use when hiring a CHW, information on a variety of training programs, various resources on chronic diseases, and examples of CHW-related policies.

This resource is hosted on Dropbox, but is available to anyone for download. Dropbox users can also save materials directly to their Dropbox account.

IPHCA, funded by HRSA, is a membership organization comprised of Illinois community health centers. Their members provide primary care services to over 1.4 million patients each year. Members have access to educational events, networking opportunities and technical assistance from the IPHCA staff.
