Bilingual Patient Navigator – Carthage

Memorial Hospital in Carthage is hiring a bilingual full time-patient navigator in their care coordination department.

The patient navigator will assist in screening a patient’s medical health needs and may assist with screening for mental health needs. This person collect information from patients or other team members and will keep team members apprised of both medical and behavioral health services available and/or offered to patients.

This position provides linkages to community resources or other resources that would be beneficial to the patients’ overall medical and behavioral health. The navigator will, among other duties, assist with documentation and data collection; translate English language verbal and written communication for Spanish-speaking patients/clients and their families; and coordinate, develop, and distribute translated patient education materials, medical forms, and other materials.

This person may assist the marketing department in providing outreach services and messaging to the community, clients, patients, and families.

Regular work hours are 8-5 Monday through Friday; some evening and weekend hours may be required.

Qualifications: Bilingual in English and Spanish required. High school diploma or GED required. Bachelor’s degree in social work, social services, psychology, sociology or a related field of study, or equivalent work experience, is preferred.
